Well our casters on JTV use Wirecast to stream our content over to JTV, this costs normally 500 bucks. It can be done free using FME (flashmedia encoder).
Theres an alternative that still requires FME but uses less CPU resource which is running FME via Command Line (dos) I would use it but my pc has some issues atm with it.
To capture image it depends, if your capturing footage off of your computer you will want to use either VHSCRCAP or SCFH DSF, both of which are free (but i prefer SCFH)
If your capturing video from a console your going to need extra hardware, a internal or external capture card. I use the black magic intensity because it is the best on the market for capturing HD gaming. As it is now nothing can capture 1080p at 60fps (PS3 for example) so i stream at 720p.
If you need to know more tell me and i could write a step by step guide.

FME doesn't let you stream from desktop presenter does it? If it does I can't quite figure that part out.

FME lets you use VHSCRCAP or SCFH DSF, dont use desktop presenter from wirecast it just isnt as good. Wire cast is expensive but its normally used by professional groups for making the news or other tv things. The h264 video encoder is also better then most programs.
